News broke last week that Detroit Tigers ace and two-time Cy Young winner Tarik Skubal needed elbow surgery that typically comes with a three-month timeline to return.
Skubal had that surgery on Thursday. After the procedure, MLB insider Jon Heyman reported that Skubal had a conservative timetable of two months to return and that he could conceivably be back within 4-6 weeks.
The report raised eyebrows and questions as to how that timeline was possible. On Sunday, Skubalâs agent Scott Boras revealed that Skubal underwent a new procedure that he confirmed is expected to significantly shorten his time on the shelf.
Boras told ESPNâs Buster Olney on the âBaseball Tonight Podcastâ Sunday that Skubal is the first pro athlete to undergo the procedure that he and Dr. Neal ElAttrache coined the âSkubal scope.â Per Boras, the procedure is more like âan injectionâ than a surgery.
âDr. Neal ElAttrache described essentially that he had â there's new technology where a needle-sized device, he can manipulate it where it doesn't in any way â he can put the scope in, and has a camera on the end of it that gives him a 120 degree look at everything,â Boras said.
âWhere he can be precise about where the bone chip was, and he could do this in a way that would not interrupt the tissue as a normal arthroscope would.â
Boras confirmed that Skubal didnât have any ligament damage. Instead, he had a âlima-bean sized particleâ in the back of his elbow that was an impediment to his pitching motion.
Boras declined to offer a precise timeline for Skubalâs return to pitching in a live game, but projected it could be âwell less than halfâ of whatâs normally expected from an arthroscopic procedure. With this new technology, Boras doesnât expect Skubal to have to undergo the typical rebuild of his arm readiness that he established in spring training.

Per Scott Boras, Tarik Skubal is the benefit of the latest remarkable advancement in sports medicine that could bring him back to the mound a lot sooner than expected.

âWe really feel that the buildup will be, really â he will not lose the buildup that he had, you know, prior to the surgery,â Boras continued. âAnd he'll be able to return to performance at just a much, much earlier stage.â
Boras projected that Skubal was âabout a week outâ from returning to throwing, citing in part the limited tissue damage from the new procedue.
âWe know from the mere fact that the normal tissue invasion and bleeding and swelling that goes with a customary arthroscope is not there,â Boras continued. âThis is really almost like receiving a shot. And, so, time will tell, but we think it's going to be a much shorter period.â
Dr. ElAttrache is one of the most respected and accomplished surgeons in sports medicine. And if the projections hold up, this new procedure is the latest to dramatically reduce recovery time from injuries that generally come with lengthy layoffs.
Boston Celtics All-Star Jayson Tatum underwent a then-new, minimally invasive procedure after rupturing in Achilles tendon last postseason. Per previous timelines, Tatum was expected to miss the entirety of the following NBA season.
Instead, he returned with 16 games remaining in the regular season and was able to play and play well through most of Bostonâs playoff run that ended last week. It was a remarkable recovery and one that caught the NBA by surprise.
Skubalâs injury and elbow surgery arenât as significant, but if the âSkubal scopeâ has the intended result, it could dramatically shorten a timeline that traditionally would project to sideline him through the All-Star break at the earliest and knock him out of the Cy Young race during a contract year.
Neither Skubal nor the Tigers have confirmed the the updated timeline or the nature of his procedure. But both are certainly hoping that the results are as good as Boras projects.
