Johnny Cardoso suffered a "high-grade" right ankle sprain during training with Atlético Madrid on Thursday, May 7, which could spell the end of his World Cup hopes.
The U.S. men's national team midfielder was injured two days after coming on as a 57th-minute sub in Atlético's 1-0 loss to Arsenal in the Champions League semifinal second leg.
"Johnny Cardoso was unable to complete Thursday's training session on May 7 after suffering a right ankle injury," a statement from Atlético Madrid read.
"Medical staff conducted tests, and the medical report indicates that the midfielder has a high-grade sprain of his right ankle.
"The Rojiblanco number '5' will undergo physiotherapy sessions and rehabilitation work in the gym, and the evolution of his injury will determine his return to competition."
Though the severity of the injury isn't fully clear, the fact it was classified as high-grade puts Cardoso in serious jeopardy for the World Cup.
Even a low-grade sprain can sideline players for multiple weeks, and the USMNT will release its World Cup roster on May 26.

Cardoso was already on the bubble for the World Cup squad. The midfielder has earned 23 caps for the USMNT, but has only managed to make five international appearances since the start of 2025.
Injuries have hampered the 24-year-old, who managed to play the first half of a March friendly against Belgium before departing camp with a muscle issue.
It's been a frustrating debut campaign with Atlético for Cardoso, who joined last summer after a season and a half with Real Betis.
Cardoso has been limited to 30 appearances in all competitions, missing time with multiple injuries. The midfielder's latest setback may cost him the remainder of the season, as well as a chance at his first World Cup.
