A former Wales captain has said "we need to do better" after the match programme for a Wales Women's Six Nations game contained the wrong names of some of their Scottish opponents.
Rather than listing the current team for the tournament, it printed the Scotland squad from last year's Rugby World Cup.
Phillipa Tuttiett, who is now a rugby pundit, said: "Could you ever fathom that happening in the men's game?"
Neither the Scottish or Welsh rugby unions would comment.
But the problem ahead of last weekend's match in Cardiff is understood to relate to issues around printing deadlines and up to date squad details not being available in time.
It meant that four players currently representing Scotland were missing from the programme entirely.
Demi Swann, Emily Coubrough, Holland Bogan, and Rianna Darroch all marked their debuts with a 24-19 victory over Wales at the Principality Stadium last Saturday.
The 21-year-old second-row/blindside Holland Bogan said she enjoyed an "unreal" experience.

Speaking on the Scrum V podcast, former Wales 15s and sevens captain Tuttiett said: "Could you ever fathom that happening in the men's game? Absolutely not".
"Especially if you're getting your first cap, you would keep your first programme," she said.
"It's a huge moment and I can only apologise to the three Scotland girls who got their first cap because the squad named in the official programme was the squad from last year's Rugby World Cup".
Tuttiett added: "Demi Swann, Emily Coubrough, Holland Bogan - I'm so sorry. Nothing will be able to replicate your first programme. But well done. Those particular players played really well.
"Also Rianna Darroch didn't come on but it was her first time on the bench and she didn't come on but she won't have a programme saying her name".
